Think of the annual maximum as a bucket. At the start of each plan year, the bucket is full — say, $1,500. Every time your insurance pays toward a covered service, that amount is drawn from the bucket. When the bucket is empty, your insurance stops paying for that year.

Preventive services (cleanings, exams, X-rays) are often covered at 100% and may or may not draw from the bucket depending on your plan. Basic services (fillings) are typically covered at 80%, and major services (crowns, bridges) at 50% — all drawing from the same annual maximum.

Plan year vs. calendar year: Your plan year may start January 1 or on a different date set by your employer. Check your plan documents to know when your benefits reset.
Unused benefits do not roll over: If you only use $400 of a $1,500 maximum, the remaining $1,100 is lost when the plan year ends. Use your benefits before they expire.
